Patient Forms

New patients at Southwest Michigan Eye Center are asked to complete several important forms. This information is required to ensure our Battle Creek and Southwest Michigan patients will receive the best medical care possible. Your copay is due a check-in prior to your appointment. Otherwise you may be asked to reschedule your appointment.

Please be considerate of our doctor/patient time when you are unable to keep your scheduled appointment. Kindly give 24 hours’ notice when you call to cancel your appointment or you will be assessed a $50 fee which must be paid prior to your next appointment.


Most appointments, such as an initial consultation or complete exam, take approximately 1 1/2 – 2 hours. Both eyes are dilated for complete exams, laser vision and retinal evaluations. Dilatation causes sensitivity to light and an inability to see things clearly. Those receiving dilating drops should plan on having someone else drive them home to ensure a safe trip.

Medicare and many other insurance companies will not pay for a refraction fee. This is usually an out of pocket cost to the patient; therefore payment of the $40 refraction fee at the time of service in addition to any co-payment your insurance may require.

Payment in full is required the day before surgery. For your convenience, our practice accepts cash, checks, VISA/MasterCard credit cards and CareCredit.


Southwest Michigan Eye Center participates with most medical insurance plans and many vision plans. Your medical insurance covers eye care including diagnosis, management and surgery of ocular diseases and disorders. Vision plans cover exams specific to examining the eye for the purpose of prescribing and dispensing corrective lenses and screening vision to detect certain eye abnormalities.

Our patients are responsible for all co-pays and deductibles for visits that are billed to insurance companies with whom we participate. SWMEC will process the billing for all services that are considered to be out of network to insurance companies that we do not participate with for our patients, but the patients remain responsible for all fees in full.
