Southwest
Michigan Eye Center believes every patient should expect two

Jeffrey Colquhoun, M.D., Randall C. Stout, M.D. and Larry
M. Vander Plas, M.D. offer complete local eye care with over
fifty years of combined experience. Southwest Michigan Eye
Center offers Comprehensive eye exams, LASIK vision correction
with 100 % blade-free Intralase Method™ and Wavefront , NearVision CK (Conductive Keratoplasty) , no shot
- no stitch - Cataract surgery, including the new ReSTOR® and ReZoom™ multifocal lens implant, Laser Treatments for Glaucoma and Retinal disorders, VEGF Drug Treatment of Macular Degeneration, diagnosis and care of disorders of the eye,

WHAT
YOU CAN EXPECT
During
your initial visit to Southwest Michigan Eye Center, your
eyes will be examined thoroughly. Your vision is tested with
and without corrective lenses for both distance and near.
An external eye examination is also performed, including an
evaluation of your eyelids, pupils and eye muscles function.
Through
quality care, personal service and advanced technology, we
strive to provide patients with the best care in the industry!

One of the factors that allows Southwest Michigan Eye Center
to offer our patients quality care, is our outstanding and
dedicated staff. At our practice, continued medical education
is of the utmost importance. Our Certified Ophthalmic Technicians
(C.O.T), Certified Ophthalmic Assistants (C.O.A.), Medical
Assistants (M.A.) and Ophthalmic Assistants are continuously
being re-educated on the practices, policies and procedures
of Ophthalmology.
Everyone
from the doctors and management, to our clinical and front
office staff, are here to help you in whatever way possible.

Southwest Michigan Eye Center is conveniently located on Capital
Avenue, S.W., in Brookside Medical Center, just 1 mile south
of I-94.

Hours:
Our office is open Monday -Thursday 8 am - 5 pm and
Friday 8 am - 4 pm.
Payment
& Insurance
As
a patient, you are responsible for any Southwest Michigan
Eye Center charges incurred on your behalf. Payment may be
made in the form of cash, check or credit card. We accept
Master Card, VISA and Discover.
Southwest
Michigan Eye Center will also gladly file insurance claims
for services received at our practice. We welcome Medicare,
Medicaid, Blue Cross/Blue Shield and Blue Care Network. Other
insurances that we accept include, but are not limited to,
Aetna, CNA Health Partners, Cigna, Eaton, Mailhandlers, PHP,
Unicare and United Health Care. Please be sure to inform us
of any insurance coverage you have upon arriving.
No-Show Policy
On November 1, 2006, Southwest Michigan Eye Center's no-show policy went into effect. The no-show policy states that if a patient does not show for their scheduled appointment, the patient will be assessed a $20.00 charge. If you are unable to keep a scheduled appointment with Southwest Michigan Eye Center we ask that you kindly give us 24 hour notice.
